How to Refer

How to refer

Our team are helpful, qualified and experienced to work with young people, so please don’t hesitate to give us a call.  The service is available at Level 1, 641 Mt Alexander Road, Moonee Ponds. We are available to meet young people at our office or can arrange another location such as a community centre, school, library or other public facility.

Please note sessions are by appointment only - Monday to Friday between 9am and 5pm.

Self-referral
Young people are encouraged to make contact with our service directly. You can either fill in our referral form above or give us a call if you would like to have a chat first!

Drop in
Drop in to see us! We are located at Level 1, 641 Mt Alexander Road, Moonee Ponds if you would like to enquire about the service and receive a hard copy of a referral form. Our office hours are Monday to Friday, 9am - 5pm. We cannot guarantee an appointment right away.

Service referral
Community based organisations, school wellbeing staff and other community based services can refer young people to the service. Please fill in the referral form above.

Family referral
Family and friends of young people can refer to the service ensuring the young person is aware of the referral being made and has given consent. Please fill in the referral form above or give us a call!

Secondary Consultation
We offer secondary consultation to anyone working with young people or parents/carers within the Moonee Valley region. Contact us on 92431225 and we can support you to ensure young people are receiving the most appropriate service based on their needs.
